WHAT YOU'LL DO AS OUR RN / LPN HOME HEALTH ASSESSMENT NURSE :

• Perform in-depth care assessments and develop a tailored Plan of Care for elderly clients and clients with disabilities
• Inform clients and family members about the care plan and perform regular check-ins to ensure needs are fully met
• Coordinate with physicians, managers, home care aides, social workers, and other members of the nursing team to ensure we follow through with the Plan of Care
• Maintain accurate, timely, and updated documentation
• Participate in case conferences with other care-related disciplines to build connections and develop your knowledge

WHAT WE'RE OFFERING :
