JOB OVERVIEW

Job Title : Physician Family Medicine (Will also consider Internal Medicine)
Job Type : Full-Time, Permanent (Employed)
Location : St. John, IN
Service Setting : Outpatient
Coverage Type : Clinical + Call
Coverage Period : Ongoing
Clinical Shift Schedule : Monday Friday
On-Call Shift Schedule : 1 : 4 call rotation
Call Type : Rotational
Call Ratio : 1 : 4

PATIENT INFORMATION

Patient Demographics : General family medicine patients

Phone Consults : Yes

Case Mix : General outpatient family medicine

FACILITY INFORMATION

EMR System : EPIC fully integrated HIMSS Stage VII

Office Equipment Available : Standard primary care resources

Specialty Backup Available : Collegial group of system physicians

Support Staff Available : Nurse practitioner support in outpatient settings

Reason for Coverage : Growth and expansion of physician service lines

PRIVILEGES & COMPLIANCE

Credentialing Timeline : days post-offer

REQUIRED PROCEDURES

General outpatient family medicine care

JOB REQUIREMENTS

Licenses : Active IN medical license (or ability to obtain)

Certifications : Board Certified or Board Eligible in Family Medicine or Internal Medicine

Experience : Open to all experience levels

Other Qualifications : EMR proficiency (EPIC); supportive group practice structure

DUTIES & RESPONSIBILITIES

Deliver comprehensive outpatient family medicine services

Participate in 1 : 4 call rotation

Collaborate with system physicians in a collegial work environment

Use EPIC EMR system for all clinical documentation

Contribute to system-wide initiatives and quality improvement programs
