Responsibilities :

Conduct comprehensive patient assessments to develop and implement individualized care plans

Collaborate with the interdisciplinary team to ensure seamless care delivery and coordination

Document patient progress and maintain accurate records in accordance with regulatory standards

Implement and monitor patient care plans, addressing needs and concerns in a timely and effective manner

Provide education and support to residents, families, and healthcare professionals

Participate in quality improvement initiatives and performance improvement activities

Requirements :

Current LPN or RN license in the state of Florida

Minimum of 0 years of experience in a long-term care setting (new graduates welcome to apply!)

Strong communication and interpersonal skills

Ability to work effectively in a fast-paced environment and prioritize tasks

Compassionate and empathetic approach to patient care
