Responsibilities

Job Summary :

Leads the delivery of quality patient care and adherence to policies, procedures and case management

protocols.

Responsibilities :

  • Actively participates in the process of monitoring, guiding and evaluating nursing staff performance.
  • Responsible for coordinating activities, prioritizing workflow, and promoting decision-making and
  • problem solving.

  • Serve as a first point of escalation for team members needing consultation for and / or management of patients requiring complex or advanced treatment / care.
  • Provide onboarding support to new associates. Function as a preceptor to new associates and works with management to develop and adjust the orientation plan.
  • Provide input into hiring, training, directing, development and evaluation of staff as well a
  • day-to-day operational support to management as needed.

    Requirements

    Licensure / Certification / Registration :

  • Required Credential(s) :
  • Registered Nurse obtained prior to hire date or job transfer date.
  • Preferred Credential(s) :
  • Case Manager credentialed from the American Case Management Association (ACMA).
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
  • Education :

  • High school diploma / GED with 2 years of experience, or Associate's degree, or Technical degree required. Master's degree preferred.
