Position Summary :
- The Intensive Care Coordinator (ICC) will provide intensive care coordination to children, youth, their families and their interagency teams on assigned cases within the Montgomery County Office of Children and Youth.
- The ICC process is to support children, youth and their families to collaborate with their interagency team and to address barriers to progress and reduce further penetration into the system.
- The ICC will provide individualized support and planning in a manner that is collaborative, person-centered, youth and family driven, strengths-based, transparent, culturally responsive, and proactive.
- Keeping families informed and connected, the ICC will serve as a mechanism for supporting the professionals in working together with families in a solution-based manner.
- Coordination assignments will be referred by Montgomery County OCY and are intended to assist a youth, their family and team address a lack of progress or barriers to planning on a temporary basis.
Responsibilities :
Provide Intensive Care Coordination to assigned families that is collaborative, person-centered, youth and family driven, strengths-based, transparent, culturally responsive, and proactive.Meet face to face with youth and family as needed, at home, school, shelter, detention center or the community and will contact family, treatment team, teachers, health care providers and natural supports, or via phone / email, telehealth as appropriate.Act as the lead planning agent for assigned youth within the ICC program, providing leadership for the ITM planning process.Actively collaborate with Montgomery County Children and Youth staff and leadership to develop a collaborative partnership in engaging assigned youth and families.Prep, plan and debrief with all team members around the ITM meeting for assigned youth.Be on site at OCY offices 1-2 days per week as assigned to ensure collaboration and effective team building.Document all efforts in agency EHR.Prepare reports as requested to supervisor and ITM team for each assigned youth.Proactively address barriers to the planning process with supervisor, ITM team and OCY staff.Celebrate successes with team by acknowledging and highlighting above and beyond efforts of team members.Actively plan for discharge from this level of care coordination and ensure appropriate level of care and support is in place with a warm hand off prior to discharge.Receive training in various person-centered planning models as assigned.Perform additional duties or tasks, as assigned, or requested.Required Qualifications :
Master’s degree in social services or related field 1 or more years of experience with team dynamics and systems work, e.g. family systems. 3 or more years of comprehensive child-serving systems experience.Preferred Qualifications :
Excellent case management, leadership, partnering and customer service skills.Expertise with conflict resolution and management.Receive training in High Fidelity Wraparound Care Coordination model.Work Environment :
Hybrid work environment both in a standard office environment, and remotely from home, as well as community settings to include client home, schools, court, doctor’s offices and hospitals.Compensation & Benefits :
