Job Title : Travel Licensed Practical Nurse (LPN) – Day Shift Facility : Community Rehabilitation Hospital West Location : Indianapolis, IN Shift : Days

Position Overview : Community Rehabilitation Hospital West is seeking a dedicated Travel Licensed Practical Nurse (LPN) to join our team for day shift coverage. This role is ideal for a compassionate and skilled nurse who thrives in a rehabilitation setting and is committed to delivering high-quality patient care.

Key Responsibilities :

  • Provide direct patient care under the supervision of a registered nurse or physician
  • Administer medications and treatments as prescribed
  • Monitor patient health and report changes to the healthcare team
  • Assist with rehabilitation therapies and support patient recovery goals
  • Document patient care accurately and timely
  • Collaborate with interdisciplinary teams to ensure continuity of care
  • Maintain compliance with hospital policies and regulatory standards

    • Qualifications :

  • Current LPN license in the state of Indiana
  • Minimum of 2 year of clinical nursing experience preferred
  • BLS certification required
  • Strong communication and organizational skills
  • Ability to work effectively in a team-oriented environment
  • Must Have Skills / Experience :    IRF or Acute
