Your Role

Were seeking a compassionate and skilled Home Health Registered Nurse (RN) to join our growing team. In this role, youll use your clinical expertise and assessment skills to provide one-on-one care to patients in their homes, ensuring safety, comfort, and optimal recovery.

What Youll Do

• Conduct OASIS assessments and create individualized Plans of Care tailored to each patients needs
• Perform skilled nursing visits, including wound care, ostomy management, IV infusions, and PICC line care
• Educate patients and families on disease management, medications, and home care safety
• Collaborate with physicians, therapists, and other team members to ensure coordinated, patient-centered care
• Document all visits, interventions, and progress accurately and on time
• Monitor patient status and promptly report changes to the appropriate provider

What Were Looking For

• Active Florida Registered Nurse (RN) license (required)
• Home Health and OASIS assessment experience (required)
• Solid clinical background with strong critical thinking skills
• Excellent communication and time management abilities
• Current CPR / First Aid certification
• Valid drivers license and reliable transportation
