Credentialing : Begins upon completion of credentialing.

Minimum Requirements

Board Certified / Board Eligible : Gastroenterology

Certifications : ACLS (AHA), BLS (AHA), Moderate Sedation

Licensure : Active Illinois license, IL DEA, and CS required
