  • Provide compassionate and personalized nursing care to patients in their homes, ensuring their comfort and well-being.\n
  • Conduct comprehensive assessments of patients' health status and develop individualized care plans.\n
  • Administer medications, treatments, and procedures as prescribed by healthcare providers.\n
  • Monitor and evaluate patients' responses to treatment and report any changes to the appropriate medical professionals.\n
  • Educate patients and their families on disease management, medication administration, and self-care techniques.\n
  • Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to coordinate patient care and achieve optimal outcomes.\n

  • Registered Nurse (RN) license in good standing.\n
  • Previous experience in home health care or hospice care is preferred.\n
  • Strong clinical skills and knowledge of evidence-based practices in nursing.\n
  • Excellent communication and interpersonal skills.\n
  • Compassionate and empathetic attitude towards patients and their families.\n
