Job Description

Overview : The Community Health Representative (CHR) will work under the supervision of the Community Health Manager, aiding the outreach and effectiveness of health services to the Tribal community. This position is also responsible for managing the WIC (Women, Infants, and Children) program, providing Tribal Maternal Support Services (MSS) and assisting with the Tribal Diabetes Program. The CHR is expected to provide health educational and outreach support to Tribal members in such areas as conducting classes, seminars and other special projects as needed.

ESSENTIAL FUNCTIONS OF JOB :

Women, Infants and Children’s Program (WIC)

1. Manages WIC day to day functioning including caseload management and maintenance, program regulation compliance, clinic flow and materials management as WIC Coordinator and WIC breastfeeding Coordinator.

2. Communicates with State staff and disseminates information from the State WIC program to the local agency’s staff.

3. Collaborates with WIC Registered Dietitian to develop, conduct and evaluate annual Nutrition Education Plan and submits to WA State WIC Office.

4. Develops and updates Local WIC Policies as required by the State WIC Office and Lower Elwha Health Department, conducts yearly Self Monitor plan as outlined by WIC policy.

5. Acts as BF coordinator, working to improve breastfeeding initiation and duration rates by WIC clients. Promotes breastfeeding as preferred infant feeding method and providing breastfeeding education to WIC clients as Certified Lactation Educator.

6. Issues to and instruct clients in breast pump use. Maintains Breast Pumps per State of WA and Local WIC equipment loan policy.

7. Collaborate with Lower Elwha Accounting Department to complete required State budget documents.

8. Acts as Competent Professional Authority (CPA) providing WIC services including eligibility, anthropomorphic measures, hemoglobin measurement, risk assignment, outside referrals and food package determination and documents appropriately in WIC specific computer system.

9. Provides WIC education including maternal and pediatric nutrition, breastfeeding and WIC check use.

Maternity Support Services-Community Health Representative

1. Outreach to qualified individuals for participation in MSS program and assist qualified clients in application screening for MSS program.

2. Collaborates with MSS team to determine client needs, participates in monthly Case management and fulfills tasks assigned to CHR by either home or office visits.

3. Completes billing paperwork as determined by Lower Elwha MSS and maintains patient files and charts in secured area.

Diabetes Program Assistant

1. Participates in Diabetes Advisory Committee, annual Diabetes Day and other Diabetes Events.

2. Assists Diabetes Program Coordinator in record keeping, data collection and outreach.

Community Health

1. Participate in the development and implementation of cancer prevention, education and screening activities.

2. Attend weekly Community Health Meetings and collaborate in planning community health events.

3. Collaborate with Lower Elwha Health Department Registration system to enroll WIC / MSS clients into EHR system.

4. Participate in community health activities such as health fairs, cancer support groups

5. Attend monthly all-staff health department meetings and activities when appropriate.

6. Back up Transport driver for Para-transit program.

7. Other duties as assigned.

KNOWLEDGE, SKILLS, ABILITIES REQUIRED BY POSITION :

  • Knowledge of computer applications such as Microsoft Word and Excel.
  • Knowledge of health and wellness field.
  • Skilled in verbal and written communication.
  • Skilled in delivering cultural and literacy appropriate education to adults.
  • Ability to work independently and in a team.
  • Ability to self-motivate and motivate others.
  • Ability to follow departmental and Tribal policies and procedures.
  • Ability to follow instructions and adhere to prescribed routines and practices.

MINIMUM QUALIFICATIONS

  • Current State of Washington Medical Assistant Certification or eligibility within 3 months of hire.
  • Three years CHR experience
  • High School Diploma or GED.
  • Familiarity with WIC and MSS program guidelines.
  • Must pass pre-employment drug and alcohol testing.
  • Must obtain HIPAA, CPR and First Aid training within 30 days of employment.
  • PHYSICAL REQUIREMENTS

  • Requires standing, sitting, stooping and bending intermittently.
  • Must be able to lift greater than 10 pounds.
  • Requires ability to hear and understand verbal instruction.
  • Requires use of standard office equipment such as computers, telephones, fax machines and photocopiers.
  • CONDITIONS OF EMPLOYMENT :

  • Must maintain State of WA Medical Assistant Certification for WIC.
  • Must demonstrate awareness and support of the Health Services Department mission, vision and core values.
