Requirements : Experienced Family Nurse Practitioner or Physician Assistant, with North Carolina State License

Location : Lincolnton, NC, at Lincoln Internal Medicine, a practice present for nearly 30 years in the community

Patient Demographics : Adults (age 18 years and older)

• Maximum of 40 hours per week (including breaks and administrative time)
• Expected average patient load of 2 visits per hour
• Collaborative environment with onsite lab, behavioral health therapists, and nutritionists
• Innovative, integrated use of telehealth
• Centralized care coordination, care management, and population health management
