Job Title : Advanced Practice Clinician – Nurse Practitioner / Physician Assistant
Location : Skagit, Whatcom, and Northern Snohomish Counties, WA
Duration : 4–6 months
Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM
Travel : Local travel during shift between multiple facilities
Position Summary :
Seeking a dedicated Advanced Practice Clinician (APC) to provide integrated, longitudinal care to high-risk members in senior living settings. This role is part of an interdisciplinary team focused on improving outcomes and quality of life for patients with complex medical needs.
Primary Responsibilities :
- Conduct comprehensive clinical assessments
- Manage acute and chronic medical and behavioral conditions
- Ensure accurate and complete documentation, including ICD-10 coding
- Collaborate with interdisciplinary care teams
- Lead advanced illness and care planning conversations
- Provide education and counseling to patients and caregivers
Required Qualifications :
Active and unrestricted license in the state of residence (or ability to obtain by start date)NP : Master of Science in Nursing or Doctor of Nursing Practice from an accredited programPA : Degree from an accredited Physician Assistant programBoard certification :NP : AANP or ANCC (FNP, ANP, GNP, or AGACNP)PA : NCCPAActive DEA license / prescriptive authorityAbility to meet physical job requirements (e.g., lifting 30 lbs, navigating stairs)Reliable transportationAbility to obtain collaborative practice agreement (if required by state)Fully vaccinated for COVID-19 (per federal, state, and client requirements)Preferred Qualifications :
