Key Responsibilities

Collaborate with ACO partners to design and refine care pathways to enhance patient outcomes

Analyze post-acute care trends and performance metrics to identify improvement opportunities

Lead cross-functional teams to develop strategic initiatives aligned with ACO goals and CMS benchmarks

Required Qualifications

Bachelor's degree in healthcare administration, Public Health, Business, or related field

Clinical License preferred

7+ years of experience in healthcare operations or value-based care programs

Strong analytical skills with experience in healthcare data analysis

Familiarity with ACO models and post-acute care strategies
