Community Care Nurse Manager
The VA Puget Sound Healthcare System is recruiting for a Community Care Nurse Manager at the American Lake VA hospital in Tacoma, Washington. The Community Care Nurse Manager is a registered professional nurse who exemplifies leadership and oversees registered nurse staff within the VA Puget Sound Community Care team. The Nurse Manager is entrusted with the responsibility of oversight of nursing practice, ensuring patient care is streamlined between VA and Community Care teams, fostering interdisciplinary collaboration to optimize patient care delivery, and supporting creative approaches to managing complex client care. The Nurse Manager is responsible and accountable to the Integrated Care Coordination Director of Nursing for all elements of the nursing process. Possesses intimate knowledge of the internal referral care process, clinical review criteria, utilization management standards, clinical documentation requirements, community care clinical and administrative eligibility criteria, and Veterans Health Administration standards and guidelines. Will work collaboratively within the Office of Integrated Veteran Care to triage Request for Services to coordinate care processes defined by the IVC Field Guidebook to include developing a care coordination plan, reviewing medical documentation from community providers, communication of care coordination needs, and supporting the Veteran through the completion of the episode of care. Foster team-oriented environment among staff to work collaboratively with administrative staff and other clinical staff within the Community Care team, building trust in relationships. Will support the medical center's mission to improve timely access to care, ensure Veterans who want to receive care in the community are referred and scheduled into the community, and support the delivery of high quality, timely care to the Veteran receiving community services. Functions as a leader and collaborates with a multi-service and multi-disciplinary team within the local VA facility and in the community. Utilizes dashboards, databases, and various applications to monitor, review, and present data with the purposes of streamlining care and ensuring nurses are functioning at the top of their license. Leads the provision of nursing care to patients with complex care needs utilizing the nursing process in alignment with the ANA Standards of Nursing Practice. Applies clinical knowledge / judgment to lead staff involvement in planning, decision-making, and evaluating outcomes. Improves Veteran / patient driven care by resolving experience issues such as coordination of care, timeliness or process issues, common themes of dissatisfaction, or environment of care concerns for the immediate practice setting. Revises work schedule and assignments on short notice to meet changes in prescribed workload and deadlines. Provides guidance to all assigned staff.
