Geriatrics Nurse Practitioner

Details :

• Setting : hybrid remote and in-patients' homes role
• Location : Harrisburg / Hershey, PA
• Schedule : Mon-Fri (8a-5p includes travel time for in-home visits)
• Duties :
• APP's serve multiple serious conditions like CKD and / or ESRD / ESKD
• Conducts assessments including annual wellness exams
• Counsels and educates patients and their families about programs
• Responsible for coordination of care with primary care providers
• Part of the Integrated care team working with RN and Social Worker
• Prescribe medications, order tests, collaborate with Monogram physician
• Follow up and evaluate patient's status
