Responsibilities Home health setting serving adult and geriatric populations with orthopedic, neurological, and functional mobility diagnoses Average caseload of 4.5 patient visits per day Contract is 13 weeks in length Monday through Friday, 8 : 00 AM to 5 : 30 PM, with occasional rotation to work one Saturday every 6-8 weeks (Saturday worked is balanced by a weekday off) Occasional travel to central and northern regions is required Mileage is reimbursed at the government approved rate Uses NetSmart EMR for documentation and a laptop is provided for point of service documentation Cell phone used for GPS and communication Responsibilities include evaluating, treating, and planning patient care according to Physical Therapy standards Productivity expectations are an average of 4.5 visits per day Documentation must meet regulatory and reimbursement standards Applicants should be resilient, adaptable, and personable Scrub colors are flexible or business casual attire is permitted 36 hours of unbillable low census hours may be assigned per contract Travelers may be on call for low census hours; call back pay applies if called into work Radius rule : candidates must live at least 50 miles from the facility Requirements Active Wisconsin Physical Therapy license required; license required if applicable AHA BLS certification required NPI number is required; must initiate application process if not already obtained Master’s degree in Physical Therapy required Previous experience in home health settings required Experience working with adult and geriatric patients with a variety of orthopedic and neurological conditions Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts American Traveler Job ID P-629214.
