Under the supervision of the Clinical Director, as a part of the primary care team, Primary Therapist is responsible for providing standardized individual, family and group therapy as well as some case management services for our patient population with moderate and chronic mental health conditions and / or substance abuse disorders. Clinician will work to address other social needs of patients and will utilize area referral sources for needed specialty care, hospitalization, or advance psychiatric assessments and treatments.
- Role and Responsibilities
- Provide patient with minimum twice a week 1 : 1 psychotherapy utilizing professional counseling standards.
- Provide minimum 5-10 hours week Group Therapy facilitation, utilizing a combination of assigned curriculum and custom topics.
- Completes initial Biopsychosocial (BPS) Assessment and assigns diagnosis codes within 24 hours from admission.
- Completes initial individual treatment plan within 1st five days of admission.
- Continues ongoing re-evaluation of treatment as patient needs / goals change
- Initiates initial discharge plan, coordinates with case management for discharge plan finalization.
- Completes discharge summary within 5 days of discharge.
- Consults with medical and psychiatric provider / team regarding complex patient needs.
- Identify patients with urgent mental health conditions (e.g. suicidal, homicidal, psychotic, etc.) and make appropriate referrals to available emergency or psychiatric treatment facilities.
- Provide written documentation of individual and group psychotherapy sessions, assessments, interventions, and recommendations for providers and to meet requirements for industry best practice.
- Provide non-scheduled urgent assessment or crisis intervention for patients experiencing crisis.
- Provides group and family therapy session as indicated.
- Provides weekly updates to individuals involved in the patient care such as Family members, legal contacts, referral source, social or familiar coordinators (tribe, interventionists, etc)
- Participate in a team management approach to patient care / treatment.
- Maintain appropriate documentation of clinical and psychotherapy services, including all necessary updates requested by the utilization department. Ensure accurate and consistent use of encounter forms for all client contract.
- Maintain and update professional knowledge and proficiency through continuing education and staff meetings
- Perform miscellaneous job-related duties as assigned.
- Qualifications and Education Requirements
Master's degree in Counseling, Marriage and Family Therapy, Substance Abuse Counseling, Social Work.
Current (and in good standing) license as a Licensed Independent Substance Abuse Counselor (LISAC), Licensed Associate Substance Abuse Counselor (LASAC), Licensed Associate Marriage and Family Therapist (LAMFT), Licensed Marriage and Family Therapist (LMFT), Licensed Associate Counselor (LAC), Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) by Arizona Board of Behavioral Health Examiners .
1+ years behavioral health experience, possess demonstrated commitment to customer service.
Professional experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred.
