It offers 24-hour emergency services and has partnered with St. Louis Children's Hospital to provide world-class pediatric emergency care.\n\nProgress West Hospital also provides surgical services, endoscopy services, a cardiac catheterization lab, medical / surgical inpatient services, diagnostic imaging, lab services, and the Advanced Wound Center, a specialty wound treatment facility with state-of-the-art equipment. Progress West Hospital recently completed an expansion of its Childbirth Center to include enhanced services for at-risk mothers and babies, with a newborn ICU that provides care for babies born as early as 32 weeks.\n\nProgress West Hospital is a Certified Level II Stroke Center. The hospital's medical office building houses offices for specialists in primary care, cardiology, obstetrics, pediatrics, and orthopedics.\n\nPreferred Qualifications\n\nRole Purpose\n\nCoordinates the health care of select patient populations / designated service across the continuum of care. Responsible for coordination of patient contact, patient education, prepares patient / family for procedure / test in collaboration with the healthcare team.\n\nResponsibilities\n
• Facilitates patient care by assessing and evaluating patient's physical, psychosocial and emotional needs.\n
• Coordinates the delivery of safe and appropriate patient care governed by evidence-based clinical practice.\n
• Monitors care and data to optimize patient outcomes.\n
• Facilitate the education of patients and families. Acts as a resource to the health care team.\n
• Ensures actions meet and support the overall goals of the business.\n\nMinimum Requirements\n\nEducation\n
