ACLD's Job Coach provides on-site job coaching and advocacy for people we support in the intensive and follow-along phases of Supported Employment.

In this position, you will :

• Provide on-site job coaching for designated caseload
• Complete and ensure accurate and timely documentation of SAP plans, case notes, monthlies, DDPs and other required documentation.
• Conduct observational assessments on site in order to assist the person supported in learning the tasks of the job
• Travel train on the public transportation system and / or roadways
• Maintain case files for each person supported.
• Prepare all documentation for ACCES and OPWDD on a monthly basis
• Act as a liaison between the person supported, family and employer
• Attend all staff meetings, Life Plans, conferences and annual training requirements
• Provide overall assistance as assigned
