Home Health Scheduler Team Coordinator
Aveanna Healthcare has grown tremendously during the past year through a series of exciting acquisitions and we are currently looking for a Team Coordinator to join our dynamic home health care team. The Team Coordinator will support the clinical operations and management staff members.
Hours : Monday - Friday 8 : 00am - 5 : 00pm onsite
Essential Job Functions :
- Greet and assist clients, employees, and general public when entering office providing excellent client and employee customer service
- Provide daily analytical support and validation through our EMR system (Homecare Homebase) per industry regulations
- Daily activities can include but are not limited to : scheduling clinicians, notify physicians of care changes, perform pre-bill auditing
- Send patient instructions
- Verify receipt of proper documentation and follow up as necessary with clinicians, clients and families
- Manage office calls, correspondence and meetings
- Monitor office supply levels
- Provide customer support
- Support ongoing client and employee requests including staff scheduling, preparing client materials, administrative tasks and processing of client files in alignment of government / health care standards
- Work collaboratively with all branches and home office to ensure seamless coordination and service amongst our business and clinical delivery team
- Participate in staff meetings and meets regularly with branch management for individual supervision to meet performance objectives
- Comply with HIPAA and Data Privacy Practices and regulation
