Details Department : HH Therapeutic Services Schedule : 8-hour DAY shift. Monday - Friday with On-Call and Holiday Rotation. Responsibilities Provide direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization. Implement and monitor patient care plans. Monitor, record, and communicate patient condition as appropriate. Serve as a primary coordinator of all disciplines for well-coordinated patient care. Note and carry out physician and nursing orders. Assess and coordinate patient's discharge planning needs with members of the healthcare team. Requirements Licensure / Certification / Registration : BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date required. Registered Nurse credentialed from the Illinois Board of Nursing obtained prior to hire date or job transfer date required. Education Diploma from an accredited school / college of nursing OR Required professional licensure at time of hire. Additional Preferences 2 years of hospital or home health experience strongly preferred. Requires holiday and on call rotation 4 - 6 times a month (5PM-8AM - urgent issues). Live within 30-40 minutes Sharp assessment skills, critical thinking, organizational skills, excellent communication skills and some computer knowledge. This position allows for some flexibility with the exception of meeting 40 credits per week. This position is a salary plus model any credits over the expected 40 credits will be paid times your base rate. Documentation is encouraged using point care which is documenting as much as possible in the home. In some cases documentation will need to be completed after the visit. Calling patients the evening before is an expectation. Holiday, weekend and on call rotation will be a requirement. Territory includes but not limited to Arlington Heights, Elk Grove, Addison, Wood Dale, and Itasca.
