Director of Nursing
Qualifications / Educational Requirements :
- Resides in North Carolina
- Current CPR certification
- Comfortable Using Microsoft Suite
- Excellent oral and written communication skills
- Valid Driver's License with reliable transportation
- Knowledge of home care principles, practices, and regulations in North Carolina
- Graduate of an accredited nursing program (Associate's or Bachelor's degree in Nursing)
- Ability to work with and have sensitivity to the needs of the geriatric population (aged 60+)
- Must hold a current, unencumbered Registered Nurse (RN) license in the state of North Carolina
- Two (2) years' experience with the AxisCare software system preferred or similar Home Health software
- Minimum of one year of clinical nursing experience, preferably in home health, hospice, or community health settings
Client Assessment and Care
Perform comprehensive nursing assessments to identify the physical, psychosocial, and environmental needs of clients.Develop and implement individualized care plans based on the assessment findings, client needs, and physician's orders.Monitoring and Evaluation
Monitor client progress and response to care, making necessary adjustments to the care plan and interventions.Document client care and progress accurately and timely manner, maintaining compliance with company policies and state regulations.Client and Family Education
Educate clients and their families about disease processes, treatments, medications, and self-care techniques to promote independence and safety in the home.Provide guidance and support to caregivers to enhance client care.Coordination of Care
Collaborate with the interdisciplinary team, including physicians, therapists, social workers, and other healthcare professionals, to coordinate care and ensure a holistic approach to client health.Communicate effectively with other members of the care team to ensure continuity and quality of care.Compliance and Documentation
Maintain compliance with all applicable North Carolina state statutes and regulations governing home care services.Ensure all documentation is accurate, complete, and submitted in a timely manner according to agency and regulatory standards.Report significant change in client status to the Director of Nursing promptly, consistent with the client's needs.Emergency and On-Call Duties
Respond promptly to client emergencies and provide appropriate interventions.Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for home health nursing.Why Join Our Team?
