    • Your Role as a Hospice Nurse

    As an Admission RN, you will play a crucial role in our patients' care journey. Your responsibilities will include :

  • Conducting initial assessments of patients’ physical and psycho-social status.
  • Collaborating with the Interdisciplinary Group (IDG), patients, families, and attending physicians to develop care plans.
  • Providing education and interventions to manage pain and other physical symptoms effectively.
  • Ensuring all necessary equipment, supplies, and medications are available for patient care.
  • Adapting care plans as patients’ and families’ needs evolve.
  • Documenting patient and family services accurately and promptly.
  • Participating in regular IDG meetings to enhance patient care.

    • What We’re Looking For

    To thrive in this role, you should meet the following qualifications :

  • Registered Nurse : Must hold an active, unrestricted license in the State of Michigan.
  • Experience : Community health and a broad range of Medical-Surgical experience preferred.
  • Transportation : Reliable transportation with a valid state driver's license and current car insurance.
