We are currently looking for a Peer Health Navigator to join our Behavior Health Care Coordination (BHCC) program located in Jamaica (Queens), NY.
Salary : $40,000 annually, $21.97 / Hr.
Position Summary
The Peer Health Navigator (PHN) is a full-time position. This staff is responsible for working with care coordination members in developing a plan to support recovery and effectively manage future crises as they occur. The PHN will conduct outreach and enrollment and facilitate linkages to available community-based support that address physical health, behavioral health and social determinants of health. The PHN will work as part of the care coordination team that provides community support to individuals experiencing behavioral, medical, and economic crises.
