Overview

We are seeking a dedicatedIntake & Assessment Coordinatorto be the first point of engagement for youth and families entering our early intervention and prevention program. This role is central to ensuring each youth receives the right level of support, from basic needs referrals to more intensive care planning and coordination.

Job Description

The coordinator will conduct comprehensive screenings to evaluate basic needs, safety concerns, risk factors, referral source information, and eligibility for services. Assessments will also capture the involvement of family and support networks.

Based on the findings, the coordinator will determinetier placement(14) or discharge if services are not needed :

  • Tiers 12 : Light-touch coordination and referrals to stabilize immediate needs.
  • Tiers 34 : Intensive care planning, referrals to mental health and substance use services, and participation in life skills groups.

Target Population

Youth ages818in New Castle County, DE, who exhibit early behavioral health needs, family instability, and educational challenges that place them at increased risk for juvenile justice involvement. Services prioritize those experiencing school disengagement, trauma exposure, caregiver stress, or emerging mental health concernsemphasizing early prevention and system avoidance.

Key Responsibilities

  • Conduct intake assessments and screenings for youth and families.
  • Evaluate basic needs, safety, risk factors, and family / support involvement.
  • Determine service eligibility and appropriate tier placement.
  • Use assessment tools to establish a baseline for individualized planning.
  • Collaborate with community resource specialists and other team members for ongoing support.
  • Track participant progress and adjust tier placement as needed.
  • Provide referrals to relevant community-based, behavioral health, and educational services.
  • Qualifications

  • Bachelors degree in social work, psychology, human services, or related field
  • Experience in youth services, behavioral health, or family support programs.
  • Strong interviewing and assessment skills.
  • Knowledge of local resources, prevention programs, and early intervention strategies.
  • Excellent communication, documentation, and organizational skills.
