Crystal Lake Healthcare & Rehabilitation is seeking a dedicated Speech Language Pathologist (SLP) to join our growing rehabilitation team at our Bayville, NJ 08721 location. This is an excellent opportunity to work in a supportive, resident-centered environment under a highly experienced leadership team, including our new Administrator, Director of Rehabilitation, and Director of Nursing.

We are hiring for Full-Time or PRN SLP positions.

Position Summary

As a Speech Language Pathologist , you will assess, diagnose, and treat speech, language, cognitive, and swallowing disorders in adult and geriatric residents within our skilled nursing facility. You will be an essential member of our interdisciplinary team, helping residents improve communication, safety, and overall quality of life.

Key Responsibilities

Conduct comprehensive patient evaluations

Develop individualized treatment plans aligned with resident goals

Provide speech therapy using evidence-based interventions

Document all assessments, progress notes, and therapy outcomes

Collaborate with nursing, rehab, families, and physicians on care plans

Adjust treatment plans based on progress and clinical judgment

Ensure compliance with state, federal, and facility regulations

Requirements

Degree in Speech-Language Pathology from an accredited program

Active New Jersey SLP license

Strong clinical assessment and treatment skills

Solid understanding of speech / language anatomy and physiology

Excellent communication and documentation abilities

Long-term care experience preferred, but not required
