Willoughby Post Acute is seeking a dedicated and compassionate Physical Therapist to join our healthcare team!

Job Location : Willoughby, OH

Job Overview

• Evaluate patients within 24 hours of physician referral and document any delays.
• Develop and implement treatment plans per physician orders.
• Supervise Physical Therapy Assistants and ensure compliance with state practice acts.
• Communicate patient progress and plans with supervisors and healthcare team members.
• Participate in care conferences, staff training, and documentation per policy and payer requirements.
• Provide caregiver training and assist with discharge planning.
• Recommend and coordinate durable medical equipment for patient mobility.
• Maintain equipment and ensure safe, effective operation.
