About The Position

• Monday - Friday, 8am - 5pm schedule
• 12 - 15 patients per day
• Hybrid role 3 days field work, 2 days telehealth
• Primary care clinic setting with home health visits
• Geriatric patient population focus
• 3 years adult primary care experience required
• Telehealth experience required
• Independent practice with home visit responsibilities
• We provide complimentary housing and travel
