Job Summary :

We are currently seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) for a per diem (PRN) position at a skilled nursing facility in Gloucester, MA. Key Responsibilities :

• Provide direct patient care in accordance with individualized care plans
• Administer medications and perform treatments as prescribed by physicians
• Monitor and document changes in residents' conditions
• Assist with admissions, discharges, and transfers
• Maintain accurate and timely documentation in the electronic medical record (EMR) system
• Work closely with the interdisciplinary team to achieve quality care outcomes
• Promote a safe, clean, and supportive environment for residents
