Position Summary
Reporting directly to the Director of Operations and Division Chief, the Clinical Coordinator II - Registered Nurse assists providers with patient care, health education / promotion, electronic medical record (EMR) chart management, coordination of patient care, and clinical protocols as needed in the Department of Urology at Columbia University Medical Center.
Responsibilities
Assesses and collects data related to patient health status, including urological assessments such as bladder scans and wound evaluations, and inputs this medical information accurately into the EMR as instructed by the physician.
Works with the care team to develop and coordinate the plan of care across the continuum (e.g., inpatient, outpatient, home, skilled nursing facility). This includes assisting with medication management (e.g., medication renewals, medical prior authorizations for urology-specific medications) and administering immunizations and other medications relevant to urological conditions.
Supports ongoing clinical functions of the service to provide compassionate, patient-centered care. This includes handling telephone, MyChart, and in-person triage for symptom management related to urological conditions (e.g., urinary tract infections, kidney stones, post-operative discomfort) and providing comprehensive patient education on various urological topics and treatments.
Assists with the management of urgent and emergent patient situations in conjunction with care team leadership, such as acute urinary retention or post-surgical complications.
Coordinates multidisciplinary healthcare services and arranges diagnostic tests crucial for urology, including labs (e.g., PSA, urine cultures) and imaging (e.g., KUB X-rays, ultrasounds, CT scans), as ordered by physicians. Performs diagnostic, therapeutic, and interventional procedures as needed under physician supervision, which may involve assisting with cystoscopies or prostate biopsies.
Involved in procedures or first assists in procedures , which may include, but are not limited to, the following :
Reviews and acts on test results pertinent to urology (e.g., lab, x-ray), communicating findings to the physician and patients as appropriate.
Reviews and manages communications from internal and external "customers," inclusive of in-basket messages, updating immunizations, and updating health maintenance items relevant to urological health.
Ensures infection control compliance within the practice, particularly given the nature of urological procedures.
Addresses appropriate measures to provide patient and family satisfaction.
