Competitive salary range : $55k - $65k annually

Responsibilities :

Provide home-based care to patients under RN supervision

Work with a team to deliver quality patient care

Take vitals, do assessments, and document progress

Assist in carrying out patient's plan of care and educate family members

Collaborate with interdisciplinary team to coordinate patient care

Requirements :

Active LPN license in Michigan

At least one year of clinical experience as an LPN

Current CPR Certification

Reliable transportation with valid driver's license and current car insurance
