Key Responsibilities :

• Perform patient assessments and develop individualized care plans
• Provide direct patient care, including wound care, IV therapy, and medication management
• Ensure accurate and timely OASIS documentation using Homecare Homebase
• Educate patients and caregivers on care plans and home health best practices
• Participate in on-call rotations as needed
