Director of Social Services – The Willows

Oakmont, PA – Full-time; Salaried, Exempt

The Director of Social Services utilizes core principles of care coordination to oversee the social work department. The role collaborates with an interdisciplinary team (including external providers) to coordinate care while advocating for the patient / family / resident and works to improve quality of care, resident / patient experience, and the health of individuals.  The primary focus is on social determinants of health that impact resident wellness.

Requirements for the Social Services Director :

  • Have a master’s degree in social work from an accredited college or university and at least 2 years of post-acute or long-term care experience; Social Work licensure (LSW / LSCW) in PA is preferred.
  • Carry out supervisory responsibilities for the Social Services department in accordance with the policies of Presbyterian SeniorCare Network and all applicable laws.
  • Oversee interviewing, hiring, and training of team members, including planning, assigning and directing daily work, appraising performance, rewarding and disciplining team members, and addressing concerns and solving problems.
  • Possess strong interpersonal skills to support, advise, coach, and influence others.
  • Have demonstrated knowledge of Point Click Care or other HER clinical software.
  • Be proficient with PC and Microsoft Office applications, including Outlook, Excel, and Word.
