Job Description :

• Carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals.
• Maintain a positive working relationship with other health professionals and ancillary staff.
• Provide direct patient care, evaluate functional needs and outcomes, and consult with other specialists.
• Review patients' functional needs and adjust care plans to ensure optimal patient outcomes.
• Design and implement a plan of care for patients based on a thorough assessment of potential development, incorporating participation by significant others whenever appropriate.
• Conduct therapy practice within defined standards of care.
• Direct the assistance of support staff to achieve patients' functional goals.
• Communicate effectively with other professional and support staff to achieve positive patient outcomes.
• Adhere to hospital and departmental policies and procedures, quality improvement programs, safety, environmental, and infection control standards.
• Document evaluations, treatment goals, and plans. Regularly update documentation and maintain prescriptions and signatures.
• Enhance professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops.
• Keep up to date on professional literature.
• Provide direction and leadership to staff and act as a resource to promote physical therapy in the community.
• Demonstrate the ability to remain calm in emergency or crisis situations.

Additional Information : The facility reserves the right to call off travel staff up to three shifts during any 13-week contract without being held to the guaranteed hours for the week in which they are called off. Bed size : 25 beds in Critical Access Hospital, 36 beds in Extended Care. Electronic Medical Records Used : Cerner. Holiday coverage is not required.
