Job descriptionLMSW or LCSW Masters in social work required. 2+ years of experience working with children and families preferred.
Description
Responsibilities :
- Preform the initial intake for therapy / counseling.
- Be able assist patient and collaborate with providers with resources regarding social determinants of health such as food, housing, transportation.
- Be able to assist patients and collaborate with providers with resources regarding Medicaid, Medicare, insurance, legal services, medical needs.
- Work with providers and therapists to ensure patients have a current DPOA, guardianship, and the current parent plan for patient being seen in the clinic.
- Have a current bonding for being a notary.
- Provides brief problem-solving therapy or counseling services to patients.
- Be a support to family members dealing with mental illness in the family.
- Conducts interviews to determine the appropriate services to offer mental health patients and their families.
- Develops care plans that include counseling, support services, treatment methods and referrals.
- Responsible for timely and accurate completion of medical records, including assessments, progress notes outcome measures and billing entries.
- Maintains patient records, prepares reports, and monitors progress toward treatment goals.
- Performs other duties and responsibilities as assigned.
