On-Call Weekend Registered Nurse

After Hours Hospice Weekend RN (Friday 5pm to Monday 8am)\n\nWe are currently recruiting for a Hospice RN in the Dallas area to join our hospice's after-hours nursing team. The working hours are nights on Friday 5pm to Monday 8am. If there are no needs, you are free to spend your time how you like, as long you are able to commute to a care need in a timely manner. The position is salaried.\n\nEssential Job Functions / Responsibilities\n

• During the weekend be available to work Friday 5pm to Monday.\n
• Conducts patient visits and gathers information for RN Case Manager.\n
• Performs treatments, including wound care, as directed by the RN Case Manager.\n
• Assists in identifying discharge-planning needs.\n
• Reports changes in the patient's physical condition or care needs to PCP.\n
• Orders medication, DME and other supplies to be delivered to patient as needed.\n
• Participates in quality assessment and performance improvement activities.\n
• Works with Admissions Coordinator and admissions team to ensure timely evaluation and admission of patients to hospice, when admission is scheduled outside of normal business hours.\n
• With regard to admissions, ensures seamless transition of patients to hospice care by providing direct oversight of patient education and preparation for hospice care, plan of care initiation, and coordination of care with multiple service providers.\n\nPosition Qualifications\n
• RN with current licensure to practice professional nursing in the state.\n
• Demonstrates good communications, negotiation, and public relations skills.\n
• Ability to assess patient's needs and direct care to meet those needs.\n
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.\n
• Valid Driver's License.\n
• Automobile in good working order.\n
