Here's what you'll be doing

We're looking for an experienced Nurse Practitioner or Physician Assistant who can work collaboratively with our Endocrinologist and high-caliber team to deliver consistently positive outcomes for our patients. You will :

• Provide exceptional direct patient care, including
• In-person and virtual clinic visits, focusing on diabetes and obesity as well as concomitant endocrine issues
• Patient phone calls and messages;
• Remote data analysis;
• Panel-level analysis to develop care plans for high priority patients
• Call responsibilities, shared with other members of the clinical team
• Maintain constructive, trust-based relationships with local primary care physicians and other members of the team
• Engage with other members of the team in formal and informal ways to improve the health of specific patients and the overall panel

What you need to be successful

Compensation

Base salary range of $135,000–$155,000 plus equity, 20 days PTO, 7 holidays, 3 sick days, health, dental, vision, and life insurance, 12 weeks paid parental leave, 401(k) with 4% match, and 5 CME days with a $2,500 stipend and licensure reimbursement.
