Licensed Social Services Director
CareCore Health is currently seeking an experienced and compassionate Licensed Social Services Director to join our Long Term Care Facility in the Cincinnati area. The ideal candidate will be a dedicated professional with a strong background in long-term care and a passion for resident advocacy.
Responsibilities
- Oversee all social services functions within the facility
- Assist residents and families with adjustment to placement
- Provide support for emotional, social, and psychological well-being
- Develop and update individualized care plans
- Coordinate discharge planning and community resource referrals
- Ensure compliance with federal, state, and local regulations
- Lead and supervise social services staff, if applicable
- Collaborate with interdisciplinary team members to ensure holistic care
