Position Profile : This position facilitates the admission process for individuals and families enrolling in the Supportive Care for Dementia (SCD) program, providing initial assessments, education, and resources. The role involves identifying urgent needs, coordinating services, and ensuring a smooth transition to the assigned dementia educator. The goal is to support caregivers and individuals living with dementia during the initial stages of care and set them up for long-term success.
This is a hybrid role, must be local in AZ.
