About The Position

✨ Step into a leadership role focused on high-quality care for frail and elderly populations

🩺 Full-time, Monday–Friday schedule with comprehensive clinical oversight

🧓 Serve in a PACE (Program of All-Inclusive Care for the Elderly) environment—helping seniors remain independent and thriving in the community

🤝 Collaborate with interdisciplinary teams of compassionate providers

💼 Focus on clinical outcomes, quality improvement , and mentoring APPs and PCPs

💡 Responsibilities :

Conduct physicals, assessments, treatment planning, and periodic evaluations

Direct and mentor a clinical team in delivering geriatric-centered care

Lead Quality Improvement initiatives and implement care protocols

Participate in on-call coverage and interdisciplinary meetings

Collaborate closely with caregivers, families, and other professionals

Educate participants and staff on health conditions and wellness

Job Requirements

FL Active & unrestricted medical license – Required

Board Certified in Internal Medicine or Family Medicine – Geriatrics Certification preferred

DEA & CDS / CSR – Required

1+ year of experience working with frail or elderly populations – Required

Supervisory / Leadership experience – Required

Strong assessment and chronic care management skills

Committed to working in innovative, team-based care settings
