We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our growing team and deliver professional, patient-centered care in the home setting.
Position Summary
The Licensed Practical Nurse (LPN) provides skilled nursing care to clients in their homes under the supervision of a Registered Nurse or physician. The LPN will observe, assess, and support clients and caregivers, following the established plan of care and ensuring compliance with agency standards, state regulations, and physician orders.
Qualifications
- Current Florida LPN License (in good standing)
- Successful completion of Level II background screening (FDLE / FBI)
- Physical ability to perform job duties, including patient transfers and mobility support
- Knowledge of safe handling of body fluids, chemicals, and medical equipment
- Demonstrated clinical assessment skills
- Strong communication and interpersonal skills with patients, families, and staff
- Commitment to ongoing professional development and compliance with home health care standards
Essential Duties & Responsibilities
Provide assigned nursing care under the direction of an RN or physician, with supervision adjusted based on patient condition and nurse's experience.Maintain accurate and timely clinical documentation in accordance with agency policy and state regulations.Ensure all medical plans of treatment are current, properly filed, and updated as needed.Assist the RN and physician in performing specialized procedures and preparing sterile materials.Educate patients and caregivers on self-care techniques and proper home management.Administer medications and treatments as prescribed within scope of practice.Monitor, assess, and report changes in patient condition promptly to the supervising RN or physician.Participate in multidisciplinary care planning and case conferences.Maintain confidentiality and uphold HIPAA compliance at all times.Attend required in-service trainings, workshops, and continuing education programs.Participate in discharge planning under RN supervision.Perform other duties as assigned by the RN / Clinical Care Manager.Work Environment
This position requires travel to various patient homes throughout the service area. Work environments may vary depending on each client's home conditions - from clean and organized to more challenging settings. Protective equipment and training are provided to minimize exposure risks and ensure adherence to universal precautions and infection control standards.
Physical Requirements
Ability to lift or assist patients weighing up to 160 lbs, with or without assistive devices.Able to stand or move about for up to 80% of the workday.Ability to walk, climb stairs, bend, stoop, and twist as required to perform patient care.Must be able to read, write, and communicate in English clearly and effectively.Must be able to hear, see, and respond appropriately to patient needs.Mental Requirements
