Hospice RN Case Manager

Well established hospice agency has a need for an experienced Hospice RN Case Manager to see patients who reside primarily in their homes (with some patients in ALFs and nursing homes) in and around the Lowell, M?A area.




Full-time, Monday-Friday, 40 hours/week

Shorter works weeks considered

No weekends or on-call required!

Great pay rate plus mileage reimbursement




Primary nurse provides quality nursing care to identified Hospice patients. The scope of the primary Hospice RN includes direct patient care, coordination of patient and patient advocacy. Patient includes the individual, family/significant other and community. Must be comfortable with issues relative to end-of life care, death and dying.




Requires minimum 1 year of Hospice RN experience with a Medicare certified agency.
