Basic Function : Basic Function : Under the direction of the HIV and Hep C Project Director, the HIV Community Health Outreach Worker (CHW) will be responsible for providing outreach, peer support, and education services within the scope of given projects to meet the program requirements of certain grants awarded to Canyonlands Healthcare and other projects as appropriate. The position requires strong interpersonal skills to make connections and build relationships with the target patient / client base. The Community Health Worker will work collaboratively with other departments at CHC. CHW will also work alongside Community-based agencies, medical providers, primary teams to assist in coordinating care with the purpose of making critical linkages to health information and services for community members to improve patient care and outcomes.

Requirements :

Essential Functions :

Work closely with the Pharmacy Director and Grant Manager in tailoring health education and remote patient monitoring plans for each client in the cohort.
