Essential Job Duties:

• Conduct home health assessments to determine nursing equipment needs, incorporating participant preferences and goals; participate in IDT meetings to develop care plans.
• Deliver and document nursing interventions per care plans, including maintaining a safe environment, following physician orders, administering medications, and performing tests and treatments.
• Educate participants, families, and caregivers on disease processes, self-care, and prevention strategies.
• Coordinate and monitor home health schedules with the Home Health Manager or scheduling contact.
• Collaborate with the WelbeHealth Physician and IDT to ensure smooth care transitions across settings (home, hospital, SNF, etc.).

Job Requirements:

• Graduate of an Accredited School of Nursing
• Unencumbered RN License required
• Minimum of one (1) year of nursing experience in a clinical setting with a frail or elderly population, home health agency experience preferred
• Reliable means of transportation

Salary/Wage base range for this role is $51.63 - $68.06 hourly + Bonus + Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.

Compensation: $51.63 - $63.02
