Nurse Practitioner - Hospice Part-Time / PRN

Job Description

Medi Home Health & Hospice , a division of Medical Services of America Inc., currently seeks a Part-Time / PRN Family Nurse Practitioner for our Hospice location in Chesapeake, VA.

The Nurse Practitioner must have exceptional clinical skills to perform comprehensive physical assessments and documentation to support the terminal diagnosis and prognosis of end of life patients.

• Performs face-to-face encounters with the patient to determine continued eligibility for hospice care prior to the 180th day of care for re-certification, and each subsequent re-certification period.
• Documents, in detail, clinical findings as related to the hospice criteria.
• Provides clinical support to the hospice physician.
• Prepares and maintains accurate patient records and documents to support the interdisciplinary team and services provided.
• Job Requirements
• Currently licensed as a Nurse Practitioner in the state of VA .
• Current certification by a national certifying body as a Nurse Practitioner.
• Experience in hospice and palliative care preferred.
• Excellent assessment skills.
• Excellent oral and written communication skills.
