Case Manager

Job Description :

Title : School-Based Case Manager

Salary Range : $30 / hour - $31.25 / hour (DOE)

20 - 24 hours per week

Key Responsibilities :

Student and Family Services :

• Case Management : Provide trauma-informed case management to youth and families, including conducting comprehensive risk and needs assessments, creating and implementing service plans, and providing ongoing referrals and follow-up.
• Resource Connection : Link families to necessary community resources (e.g., health, mental health, social services) and help navigate any barriers to accessing these services. Provide advocacy on behalf of families as needed.
• Family Engagement and Learning : Lead and facilitate parent / guardian workshops and education sessions focused on topics such as family-school relationships, healthy parenting, conflict resolution, and child development.
• Crisis Intervention : Offer immediate support and crisis intervention services to families in urgent need, helping to stabilize situations and coordinate appropriate follow-up.
