We are currently seeking a dedicated Nurse Practitioner or Physician Assistant to join our team in Orange County, helping us extend compassionate, quality care to our community's most vulnerable populations.

Position Summary :
As a Nurse Practitioner or Physician Assistant, you will manage a caseload of elderly and medically complex patients, primarily in skilled nursing facilities. This role involves collaborating with physicians and interdisciplinary teams to develop personalized care plans, manage acute and chronic conditions, and provide proactive, compassionate care.
Key Responsibilities :
Conduct comprehensive assessments and manage a monthly caseload of approximately 260 patient visits.Develop and implement care plans in collaboration with the interdisciplinary team (IDT).Perform primary care services for frail and elderly patients in SNFs, with an emphasis on proactive health management.Manage hospital discharges and coordinate with primary and specialty care providers as needed.Provide regular in-person visits and manage care transitions and advanced care planning.Participate in rotating night and weekend on-call duties, with the flexibility to take telephonic rounds as needed.Document all assessments and care provided in EMR within facility requirements, maintaining accurate and timely records.Qualifications :
Education : Master's degree in nursing with a specialization in adult / geriatric care; valid state RN license.Certifications : Board certification as a Nurse Practitioner (adult / geriatric), DEA registration, CPR certification.Experience : Minimum of 2-3 years of experience in skilled nursing facilities or hospitalist settings. 5+ years of experience preferred.Skills : Proficiency in EMR, knowledge of SNF and hospital workflows, excellent interpersonal and communication skills.Benefits :
