SUMMARY OF POSITION :

The RN Care Manager role focuses on improving health status and connection to resources, preventative care, hospital follow up and ongoing health care for identified individuals. Collaborating with other healthcare team members, the RN Care Manager organizes, coordinates, and provides care management services to patients who are most at risk for health deterioration, sentinel events and poor outcomes. The RN Care Manager plays a multifaceted role in disease management in order to contribute to significantly improving patient outcomes and enhancing the quality of life of individuals living with chronic conditions.

MINIMUM QUALIFICATION(S) :

• Currently licensed to practice as a registered nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.
• AHA BLS prior to end of orientation period.
• Bachelor of Science in Nursing (BSN) preferred.
• Minimum of two (2) years of clinical nursing experience, with a focus on chronic disease management.
• Strong understanding of evidence-based practices in managing complex chronic health conditions.
• Excellent interpersonal, communication, and collaboration skills.
• Ability to work in a fast-paced and dynamic healthcare environment.
• Proficiency in using EMR system.
