Description :
The purpose of the Case Manager I position is to support the physician and interdisciplinary team in facilitating patient care.
The underlying objective is to enhance the quality of clinical outcomes and patient satisfaction while managing the cost of care.
This role involves providing timely and accurate information to payors.
It integrates and coordinates utilization management, patient education, care facilitation, and discharge planning functions.
The Case Manager I is accountable for a designated patient caseload.
Effective planning is essential to meet patient needs, manage length of stay, and promote efficient utilization of resources.
Responsibilities include the facilitation of precertification and payor authorization processes.
Collaboration in the management of patient care across the continuum is vital, intervening as necessary to remove barriers to timely and efficient care delivery.
Application of process improvement methodologies is expected in evaluating outcomes of care.
Support and coaching of clinical documentation efforts are key in ensuring accurate representation of severity of illness and intensity of service.
Coordinating communication with physicians is an important aspect of the role.
Knowledge and skills necessary to provide care appropriate to the patients served must be demonstrated.
Understanding the principles of growth and development relating to different life cycles is required.
Candidates should possess interpersonal communication and negotiation skills.
Analytical, data management, and PC skills are necessary.
Organizational and time management skills are needed, as evidenced by the ability to prioritize multiple tasks with minimal supervision.
Independence and sound judgment are required in interactions with physicians, payors, and patients and their families.
Preferred experience includes a minimum of three years of RN clinical experience.
Current working knowledge of discharge planning, utilization management, case management, performance improvement, and managed care reimbursement is preferred.
