Shift : Day, 5x8 (08 : 00 – 16 : 30)

Job Summary

The Physical Therapist will provide comprehensive outpatient rehabilitation services, focusing on orthopedic and pelvic floor therapy. The role requires working one-on-one with patients, conducting thorough evaluations, and developing individualized treatment plans to optimize functional outcomes.

Responsibilities

Conduct initial evaluations and reassessments for patients with orthopedic and pelvic floor conditions

Develop and implement evidence-based treatment plans to address patient needs

Provide one-on-one therapy sessions with no overlapping appointments

Utilize a private treatment room and workstation for patient care

Collaborate with physicians and other healthcare professionals to ensure continuity of care

Maintain accurate and timely documentation of patient progress and treatment plans

Educate patients and caregivers on exercises and strategies for long-term rehabilitation

Requirements

Experience : Outpatient orthopedic and pelvic floor therapy experience preferred

Licensure : Active Wisconsin or compact state Physical Therapy license required

Certifications : BLS certification required

Skills : Strong clinical assessment, manual therapy, and patient education skills

Preferred Experience : Familiarity with Epic charting system
